Please completely and accurately fill out this form to register your child.

NOTE: Students must meet the following requirements by the start of classes on 

August 24, 2020:

AGE QUALIFICATION

         i.   Pre-School:

                                               

                Playschool – 2 years old

                Nursery I – 3 years old

                Nursery II – 4 years old

                Kindergarten  – 5 years old

        ii.   Grade School:

                Grade 1   – 6 years old

                Grade 2   – 7 years old

                Grade 3   – 8 years old

                Grade 4   – 9 years old

                Grade 5   – 10 years old

                Grade 6   – 11 years old

The school reserves the right to assign and/or transfer the child in the level or grade fit for him/her, based on his/her academic capacity and maturity.

DOCUMENTARY
REQUIREMENTS

1.     Old students

        a.   Filled-up Registration Form

        b.   Original Form 138-E (Report Card)

        c.   (2) recent 1x1 picture in white background

2.    New students / Transferees*

        a.   Filled-up Registration Form

        b.   Original Form 138-E (Report Card) from the school last attended

        c.   (2) recent 1x1 picture in white background
        d.   Original PSA Certificate of Live Birth
        e.    Alien Certificate of Registration (ACR) for foreign students
        f.   Medical Records (if requested)
        g.   Other supporting documents that may be required as necessary
  
*Requirements shall be placed in a long brown envelope with the child's complete name.
